We welcome referrals from participants, families, Support Coordinators, health professionals and community organisations.

Complete the referral form with as much relevant information as possible and our team will review the request and be in touch.

Plan the support

We work with the participant and their chosen support network to develop an approach that reflects their needs and goals.

Commence support

We coordinate the support team and organise a smooth start to services.

Review and adjust

Support is reviewed over time and adapted as the participant’s needs, circumstances and goals change.

Good coordination can make a significant difference when several people are involved in someone’s support.

With the participant’s consent, Violet Haven can communicate with their nominated family members, carers, Support Coordinator or other professionals to help keep relevant information clear and support arrangements coordinated.

We respect the participant’s privacy, preferences and right to decide who is involved in discussions about their support.
